Subject: Authorization (December 23, 1949)

1. In accordance with reference (b), when notified by the Senior Member, Physical Evaluation Board, Headquarters, Third Naval District, that the necessary papers have arrived, you are authorized to report to that officer for an evaluation of your physical condition.

2. The above is authorized with the understanding that you will not be entitled to reimbursement for mileage or expense in  connection therewith. In case you do not desire to bear this expense, or, if for any reason you fail to report to the Physical Evaluation Board on the date specified, you will regard  paragraph one of this authorization as revoked.
